A Life Plan/Care Coordinator will communicate what he or she thinks best supports you throughout your life, but the final decision is always yours. Maintaining your independence is the most important facet of our program.
A Life Plan/Care Coordinator supports and guides you as you live your best life. Their goal is to help maintain your quality of life and health outcomes. They serve as your trusted partner and are there to help you set and support your wellness goals. You define what wellness means to you — whether that’s accomplishing a new task, maintaining your independence or something else entirely — your Life Plan/Care Coordination team will help get you there, serving as your navigator.
Whether you’ve got the flu, a broken bone or need round-the-clock care, your Life Plan/ Care Coordination team helps you get the information and services you need for as long as you need them. In the event of hospitalization, a member of your team will visit you as soon as possible and make sure your discharge is handled properly.
Your Life Plan/Care Coordination team also works with you to resolve any issues you may encounter during or after care.
Kendal at Home’s life plan/care coordination team is composed of professionals with diverse backgrounds in healthcare, including nursing and gerontology. Some of their credentials include:
Chris holds a master’s degree in Exercise Physiology from the University of Louisville. Her experience includes working in Cardiac and Pulmonary Rehabilitation programs and as a personal trainer working with clientele including military, medically complex and geriatric populations. She likes to read, bike, hike and participate in any activity outdoors. Chris enjoys exploring new places, pursuing new activities and loves a good joke!
Heather joins Kendal at Home with her over 11 years of nonprofit experience. Heather excels in building relationships with the aging population within the Cincinnati area for over 8 years. She carries her Bachelor’s degree in Health Administration with a focus on leadership and management. Heather lives in Cincinnati, Ohio with her husband and 2 daughters. She enjoys traveling on the weekend and spending time with her family.
Terri Lanham began her nursing career in 1985 as an LPN in Norfolk, Virginia, where she and her husband lived while he served in the United States Navy. They moved to Ohio in 1989. Terri worked for Fairview Hospital and Tri City Medicine. In 1993, she started working for Kendal at Oberlin as the health and wellness nurse in the clinic. In August of 2011, she came to Kendal at Home in the role as a Care Coordinator with 19 years of experience in the geriatric field. Terri is a professionally trained health and wellness coach. Terri resides in North Ridgeville with her husband. They have two daughters and two grandsons. Terri loves spending time with her family, traveling, and any outdoor activities. Terri’s wellness approach to life keeps her active and fit.
Larissa is excited to join the Kendal at Home team in Massachusetts. Larissa previously worked for Massachusetts aging services access points before moving to Pennsylvania and working as the Director of Social Services at a Skilled Nursing Facility. Larissa has recently returned to Massachusetts to put down roots and pursue a master’s degree in social work. She enjoys traveling with her husband and taking her two dogs for long walks.
Cheryl Menz joined the Kendal at Home team as a caregiver while pursuing her Bachelor’s Degree from Baldwin Wallace University. Since then she has graduated with a Degree in Organizational Leadership and a minor in Psychology. Cheryl brings experience in the field of caregiving and years spent working with the developmentally disabled. She resides in Avon with her fiancée and their two dogs. Cheryl enjoys spending time with her family and friends, rooting for Cleveland sports teams, and traveling, especially cruises.
Katie has worked in the field of aging since earning her master’s degree in gerontology in 1998. She has been with Kendal at Home since its beginning in 2004. Prior to joining the Kendal at Home team, Katie worked in two skilled nursing facilities and a non-medical home care agency. She completed training for wellness coaching and has extensive training in Medicare through the Ohio Senior Health Insurance Information Program (OSHIIP).
